 Fall Sports Physicals   2018-2019   


 Fall sports included are:


Football 7th-12th                                   Girls Tennis 9th-12th                

Cross Country 7th-12th                         Golf 9th-12th

Field Hockey 7th-12thn                                 Cheerleading 9th-12th                                  

Boys & Girls Soccer 7th-12th                   Girls Volleyball 9th-12th


Students interested in joining Fall Sports for the school year of 2018-2019 must pick up a PIAA physical form in the nurses office.

Students who are having their physical done by the school physician must return their completed form to the nurse by June 1, 2018.


School Sports Physicals will be done June 6th and June 13th, 2018 at 3:30 p.m. at Secondary Center Nurses Office.



June 6th 3:15 Varsity football will be done first the Jr. Varsity, Cheerleading, Cross Country, Golf Tennis, Girls Volleyball

June 13th 3:15 Soccer, Boys, GIrls first then 7th and 8th Grades,  Field Hockey


If unable to attend assigned date please attend the other date.

All junior varsity physicals will be done on scheduled dates but starting at 4pm.


Students who are having a sports physical done privately must submit their completed forms to the nurse anytime after

June 1st, 2018.

Pennsylvania School Immunization Requirements

The Pennsylvania Department of Health is changing school immunization  regulations beginning in August 2017. The regulations are intended to ensure that children attending school in the commonwealth are adequately protected against potential outbreaks of vaccine preventable diseases.
The Pennsylvania Department of Health now requires the following immunizations for entry into school and continued attendance.

Children in ALL grades (K-12) need the following immunizations for attendance:
  • 4 doses of tetanus, diphtheria and acellular pertussis* (1 dose on or after the 4th birthday)
  • 4 doses of polio (4th dose on or after 4th birthday and at least 6 months after previous dose given)
  • 2 doses of measles, mumps, rubella**
  • 3 doses of hepatitis B
  • 2 doses of varicella (chickenpox) or evidence of immunity

Seventh through 12 grade ADDITIONAL immunization requirements for attendance:

  • 2 doses meningococcal conjugate vaccine (MCV)

           -  First dose is given 11-15 years of age; a second dose is required at age 16 or entry into 12th grade.

           -  If the dose was given at 16 years of age or older, only one dose is required.

  • 1 dose of tetanus, diphtheria, acellular pertussis (Tdap)


* Usually given as DTP or DTaP or if medically advisable, DT or Td

** Usually given as MMR


Exemptions to school laws for immunizations are:

  •  medical reasons;
  •  religious beliefs; and
  •  philosophical/strong moral or ethical conviction
